Social Services

Our Social Services department here at Ellett Memorial Hospital understands how overwhelming the process is when you or a loved one is admitted to the hospital. Our Social Services department will provide information, referrals, and discharge planning services in a supportive, caring, and confidential environment.

A patient’s first encounter with Social Services takes place when the Social Service’s Designee, under the direction of the Chief Nursing Officer and licensed Social Worker, visits with the patient and/or family to initiate discharge planning.

Discharge planning begins at the very beginning of the patient’s stay. Our Social Services Designee or Patient Advocate will make sure that everything the patient and family need at the time of discharge is in place.

Our Patient Advocate will provide information or help set up services for the patient (if the patient qualifies for the services)

A patient’s care does not end upon discharge. Social Services or the nursing staff will contact the patient and/or family after the discharge to make sure the transition was a smooth one.
